Chad Krumpholz
PA-C, CAQ-Psych
I'm a Physician Assistant (PA-C) with a Certificate of Added Qualification in Psychiatry (CAQ-Psych). I work with adults, children, and adolescents. After graduating from DeSales University in 2018, my experience as a PA includes working at a general outpatient mental health clinic, an inpatient psychiatric children's hospital, an outpatient children's mental health clinic, and also embedded in an underserved pediatric clinic. I have provided consult services at a general children’s hospital and also for a state-funded mental health hotline. Prior to becoming a Physician Assistant, I worked in EMS, an emergency department, and at a court-ordered residential treatment center.
I encourage healthy lifestyle practices and good sleep hygiene habits, promote therapy as the most powerful tool we have available when helping patients long-term, and utilize a conservative medication approach in a supportive role when indicated.
Frequently asked questions
What is your cancellation policy?
Cancellations can be made up to 3 hours before the appointment time at no charge. Cancellations made within 3 hours of the appointment will incur the following fees: $25 for 1st occurrence, $50 for 2nd occurrence, and $100 for 3rd occurrence. Note 3rd occurrence may also result in potential discharge from the clinic.
How do I book an appointment?
Patients are able to self-schedule appointments.
For patients new to Mountain Mental Health, click here to book an intake appointment. Once scheduled, complete the new paperwork that will be emailed to you in order to confirm your appointment. You will receive a Zoom link for the appointment, just click on it shortly before your scheduled appointment time.
For returning patients, login to the patient portal, select "BOOK" in the upper right, then choose follow-up appointment.
What if I'm running late or am a no-show?
Your spot will be held for 5 minutes past the appointment time, at which time it will be counted as a no-show and subject to the same incremental fees noted above in the cancellation section.
Does the initial appointment mean I have been accepted as a patient?
Not always. After the initial evaluation, it may be determined that an in-person clinic or potentially higher level of care may be a more appropriate setting for you. If we are not a good fit for each other, I will provide contact information for several other clinic options.
What insurance is accepted?
Currently: Cigna and UHC. Note that insurance is pending finalization for BCBS and Kaiser. Check back soon!
What is the cost for private pay?
For patient's without insurance: Intake evaluations are $300. Follow-up visits are $150. I am able to provide an itemized receipt with appropriate billing information (super bill) if you wish to file for reimbursement for any out-of-network insurance benefits.
Do you offer crisis services?
No. Clinic services are only available during normal business hours and with 4 business hour turn-around planned for any patient requests. As a purely telemedicine service, I am not able to treat higher acuity patients (schizophrenia, bipolar disorder, personality disorders, substance abuse, eating disorders, polypharmacy, active suicidal ideations, or recent inpatient hospitalizations, etc.) and do not offer any crisis support availability. Patients needing these services should seek higher levels of care as appropriate (e.g., in-person clinics, IOP, Emergency Department, 911/988).
What is your medication policy?
If receiving medication from the clinic, it is expected that you will review the pharmacy print-out and the provided electronic summary of your medication. Also, that you keep primary care (PCP) updated on all medication changes and attend yearly PCP visits.
High risk medications that require lab monitoring are generally not prescribed at the clinic, with some exceptions for stable or pre-existing prescriptions.
Please also note that controlled substances are not prescribed from the clinic, with some exceptions for ongoing stimulant prescriptions for ADHD that does require additional steps involving your primary care provider (PCP).
Do you only offer telemedicine services?
Yes. The clinic is fully virtual, therefore there are no in-office appointments. All appointments are held using a HIPAA compliant telemedicine platform. You can connect with me from your home, work, parked vehicle, campsite, or just about anywhere you have a connection and can meet privately, without having to worry about illness or commute times.
Is the service only for Colorado residents?
Yes, you must be a resident of Colorado. Also, per current regulations, you agree to be physically located within the state of Colorado during any video visit with the clinic.
How can current patient's get in touch?
You will have access to secure messaging through the portal, link can be found at the top right of page. Patients will also have access via phone and text for non-urgent requests.
Note that that no mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties
